If the condition is of emergent or urgent nature, please contact your current physician or seek the advice of a qualified practitioner immediately

If you wish to cancel or reschedule your appointment, please notify our office 24 hours or more before your appointment. There is a $50.00 cancellation fee for missed appointments!

It is our office policy to confirm appointments with you by telephone a day prior to your appointment. If you have an answering machine or voice mail, a message will be left. Also, please bring copies (or have your primary physician mail them to us prior to your appointment) of all appropriate lab or radiologic testing to make the greatest use of your initial visit.
